We are pleased to announce that Two Rivers Public Health Department, in collaboration with Cozad Public Schools, will be making influenza vaccinations available to the Cozad students and staff this fall.
When receiving an influenza vaccine, if you have public (Medicare or Medicaid) or private health insurance coverage (such as Blue Cross/BlueShield or United Health Care), Two Rivers Public Health Department is able to bill your insurance carrier. If you have no health insurance or are underinsured, vaccinations can still be made available to your children or uninsured staff. Any in-kind donation will be appreciated in these cases but is not required. Two Rivers Public Health Department is YOUR health department and our mission is to assure health promotion, prevention, and protection for the people within Dawson, Franklin, Gosper, Harlan, Buffalo, Phelps and Kearney Counties.
Included with this letter is the federally mandated Vaccine Information Statement for influenza vaccine. To enroll for an Influenza vaccination, please complete and return the registration form – one individual per registration form – to the school office by Friday, October 20th. Please be sure to sign and date the form and attach a copy of front and back of insurance card. The health department staff will be at your schools on Wednesday, October 25th to give immunizations.
